1 · Concept overview
Established The adult brain does not lose its capacity to rewire; that capacity is actively held shut, and two of the locks are known by name. Critical periods open when a specific class of inhibitory interneuron matures, and they close when an extracellular lattice is deposited around those cells and freezes the circuit it encloses. Both mechanisms were characterised in the visual cortex and both are reversible in animals: digest the lattice with a bacterial enzyme and ocular-dominance plasticity returns in an adult rat. Frontier That result is the foundation of an engineering programme — deliberately reopen a plasticity window, then teach into it — and the honest summary of that programme is that it has produced exactly one direct human trial, in 2013, which has not been replicated in the thirteen years since and whose own second arm was null.
Established Meanwhile the field's only approved product works, and it is not a drug. Paired vagus-nerve stimulation for chronic stroke was approved by the FDA in August 2021 as a class III device: an implanted stimulator fires in time with a rehabilitation movement, and roughly twice as many implanted patients cross the clinically meaningful motor threshold as sham-stimulated patients doing the same exercises. Established The comparator is the important half of that sentence. Both arms did the physiotherapy. Every credible plasticity result in this brief has the same shape — a molecular or electrical intervention paired with an intensive behavioural one — and the intervention is measured as an increment over training that already works.
Speculative That pairing is also the structural problem. A regulator can approve a molecule or a device; it has no article to approve when half the therapy is forty sessions of supervised practice, and no reimbursement code that keeps the two together after approval. Handwave And at the exotic end the question sharpens into something no ethics framework currently addresses: if you can make an adult brain maximally impressionable for a fortnight, the drug is the easy part and the room is the intervention.
2 · Current scientific position
Established Critical periods are opened by inhibition, not by excitation. Hensch's 2005 synthesis in Nature Reviews Neuroscience established that the trigger for a cortical critical period is the maturation of parvalbumin-positive large basket cells — GABAergic interneurons whose developmental timetable tracks the onset of plasticity, whose maturation is accelerated by BDNF and gated by light-dependent signalling, and whose long axons span ocular-dominance columns. Established This inverts the folk model. The window is not a period in which excitatory neurons are unusually eager; it is a period that begins when a particular inhibitory circuit reaches a particular level of function, and it can be shifted earlier or later by manipulating that circuit. The engineering corollary follows immediately: a window with a defined opening mechanism is a window with a defined switch.
Established And they close with a physical brake that has a name and a chemistry. Perineuronal nets — lattices of chondroitin sulphate proteoglycans containing hyaluronan, link proteins and aggrecan — condense around parvalbumin cells at the end of the critical period. Fawcett, Oohashi and Pizzorusso's 2019 review states that these nets emerge around the time developmental critical periods end and are important for both their onset and their closure, and that removing them with the bacterial enzyme chondroitinase ABC restores plasticity in adult visual cortex. Established Yang and colleagues went a level deeper in 2021: chondroitin 6-sulphate, a specific sulphation state within the net, is required for plasticity and memory in ageing. Working in 20-month-old C57BL/6J mice alongside hapln1-knockout, chst3-knockout and chst3-overexpressing lines, they report that restoration of C6S levels in aged animals rescued the memory deficits and restored cortical long-term potentiation — by two independent routes, enzymatic digestion and AAV-delivered sulphotransferase. Frontier Two routes to the same rescue in one paper is a strong internal replication and a weak external one; this is a single laboratory, in mice, and nothing in the fetched record replicates it elsewhere.
Frontier The one direct human critical-period-reopening trial is small, old, and half-negative. Gervain and colleagues, including Hensch himself, ran a randomised double-blind placebo-controlled crossover of valproate plus absolute-pitch training in 2013: 24 healthy adult men enrolled, 23 completed the first arm (11 valproate, 12 placebo), 18 completed both; 500 mg/day for three days, 1,000 mg/day for eleven, 250 mg on day 15, reaching a mean blood concentration of 567 µmol/L against a therapeutic range of 350–700. Established In the first arm the valproate group scored 5.09 correct on the pitch task against 3.50 for placebo, where chance is 3; valproate was significantly above chance at t(10) = 4.08, p = 0.002 and placebo was not, at t(11) = 1.32, p = 0.21, with a one-way ANOVA of F(1,21) = 6.37, p = 0.02. Established In the second arm there was no significant difference: valproate 2.75, placebo 3.33. Frontier The authors state their own limitations, and they are severe: no baseline test was possible, the benefit appeared only in participants who received valproate first, carry-over may have impeded second-arm performance, the groups were small, and the duration of the effect was not tested. This is the most-cited human result in the entire subject, and it is a difference of 1.59 items on a nine-item pitch task in eleven men, with a failed crossover.
Established The behavioural baseline every molecule is measured against is unglamorous, effective, and largely non-transferable. Owen and colleagues ran the largest cognitive-training trial ever conducted: 11,430 completers drawn from 52,617 registrants, six weeks, a mean of 24.47 sessions. Effect sizes on the trained tasks were 0.73 to 1.63 — enormous. Effect sizes on untrained transfer tasks were 0.01 to 0.22, and the paper's conclusion is that there was no evidence for any generalized improvements in cognitive function following brain training. Established Draganski's juggling study, the structural counterpart, found grey-matter change in visual-motion areas that the authors themselves called transient and selective. Established In motor rehabilitation the equivalent tradition is constraint-induced movement therapy: restrain the unaffected limb, mass-practise the affected one, and force use. It is the standard against which stroke-recovery interventions are compared, and no primary constraint-induced trial was fetched for this brief, so no effect sizes are quoted here — but its existence is the reason the plasticity-drug literature is harder than it looks. Speculative A drug that reopens a window must beat intensive practice, in a trial where the control arm also practises intensively. That is a far higher bar than beating nothing, and it is the bar almost none of the exciting animal work has ever been asked to clear.
Established The one approved plasticity-engineering therapy pairs a device with that behavioural baseline. VNS-REHAB implanted 108 chronic-stroke patients with a vagus-nerve stimulator and randomised them, triple-blinded, to active or sham stimulation delivered in time with rehabilitation movements. About twice as many active-stimulation patients reached the clinically meaningful threshold — a Fugl-Meyer upper-extremity change of at least 6 points — as sham patients, and almost half of the active group were responders. The FDA approved the system as a class III device in August 2021. Established The manufacturer's stated mechanism is that vagal stimulation timed to movement can triple connectivity in the corticospinal tract. Frontier The arm-level Fugl-Meyer means and p-values are behind a paywall that could not be fetched for this brief, so they are not quoted here; the primary Lancet paper is named in the reading list without a link for that reason. Established What can be stated without those numbers is the structural fact that matters most: this is the first approved device whose claimed mechanism is inducing plasticity rather than substituting for lost function, and its label is a pairing. The stimulator is useless without the therapist.
Established The neuromodulation devices that are not plasticity engineering set the bar for what an implanted brain intervention has to deliver. More than 244,000 people have received deep brain stimulators, under FDA approvals for essential and Parkinsonian tremor in 1997, Parkinson's disease in 2002, dystonia in 2003, obsessive-compulsive disorder in 2009 and epilepsy in 2018. In dopamine-responsive Parkinson's at six to twelve months, subthalamic stimulation delivers roughly 50 per cent improvement in UPDRS-III against about 30 per cent for globus pallidus targeting, dyskinesia falls 64 per cent, OFF time falls 69 per cent, quality of life rises 20 per cent and levodopa dose halves. Established The complication profile is the part enthusiasts omit: intracranial haemorrhage around 5 per cent overall, of which 2 per cent symptomatic and 1 per cent causing permanent deficit or death; stroke 1 per cent; infection 8 per cent; lead fracture 8 per cent; lead migration 10 per cent; death 2 per cent. Established Medtronic's BrainSense Adaptive DBS — the first closed-loop brain stimulator, which self-adjusts to individual brain activity in real time — received US FDA approval on 24 February 2025, with the sponsor reporting an 85 per cent reduction in initial programming time from its Electrode Identifier feature. Frontier The ADAPT-PD trial's enrolment, sites and effect sizes are in a journal this brief could not fetch and are not stated here. Established Responsive neurostimulation for epilepsy has been FDA-cleared since 2013 and detects abnormal activity at a seizure focus to stimulate in response; the widely circulated one-, two-, five- and nine-year seizure-reduction percentages could not be verified against any primary source, and the manufacturer's own clinical-evidence page contains no outcome numbers at all, so none are given.
Established Psychiatry's plasticity headline and psychiatry's plasticity reality are the same modality at different sample sizes. Cole and colleagues' Stanford Neuromodulation Therapy trial randomised 29 people with treatment-resistant depression, 14 active and 15 sham, to a five-day accelerated theta-burst protocol and reported remission in 79 per cent of the active group (11 of 14) against 13 per cent of sham (2 of 15), where standard FDA-approved protocols achieve around 17 per cent remission in comparable populations. Established Goodman and colleagues ran an extended open-label accelerated intermittent theta-burst course in patients who would otherwise have received electroconvulsive therapy: acute remission 16.1 per cent in 155 completers, rising to 34.8 per cent after four weeks of tapering in 110, with 60.1 per cent mean symptom reduction in the relapse-prevention phase and 7 relapses among 43 of 61 followed. Frontier Neither number is wrong and they are not in contradiction; they are different protocols, designs and populations. But 79 per cent at n = 14 in a single site and 16.1 per cent at n = 155 in the open label is the most useful single fact about neuromodulation effect sizes available, and the authors of the smaller trial state plainly that their protocol remains to be tested directly against another active protocol.
Frontier Psychedelics are the most-funded plasticity bet with the weakest human mechanistic evidence. Calder and Hasler's 2023 review is the best-scoped account of what is actually shown. In female mice, dendritic spine formation three days after psilocybin exceeds the rate at one day, with elevated dendritic growth persisting about three days and returning to baseline by day five; pig studies show increased presynaptic density in prefrontal cortex and hippocampus after hallucinogenic doses. Frontier The proposed mechanism is an AMPA to BDNF to TrkB to mTOR positive-feedback loop, and the review states directly that the precise molecular pathways which may modify neuroplasticity after 5-HT2A receptor stimulation are not fully understood. Established In humans, peripheral BDNF results are inconclusive, there is no established dose-response relationship for human neuroplasticity, and neuroimaging connectivity changes do not directly confirm a synaptic mechanism. Speculative So the honest position is a two-level gap: rodent structure to human structure is unmeasured, and human structure to human durable behaviour change is unmeasured. What is being marketed is a bridge across both.
Frontier The most complete engineered-plasticity result in a human is n = 1, and the finding that matters happened with the machine off. Lorach and colleagues implanted a 38-year-old man with C5/C6 tetraplegia, ten years post-injury and previously enrolled in an epidural-stimulation trial, with two 64-electrode cortical implants over sensorimotor cortex and a 16-electrode paddle at the lumbosacral dorsal root entry zones driving an implantable pulse generator. After roughly six months of calibration and 40 rehabilitation sessions over 15 weeks, he walked with crutches over ramps, stairs and obstacles, stood, and controlled his legs proportionally, stably for a year including seven months of independent home use. Frontier And then the result that changes the category: the participant regained the ability to walk with crutches overground even when the brain-spine interface was switched off, with light-touch motor scores up 4 points and WISCI II rising from 6 to 16 after a three-year plateau on stimulation alone. Frontier That is a plasticity result, not a prosthesis result — durable neurological improvement measurable in the absence of the device. It is one man, and it is the only fetched demonstration in this subject of engineered plasticity producing lasting biological change in an adult human. Frontier For scale on what the same class of hardware achieves as pure decoding rather than as plasticity induction, Willett's speech neuroprosthesis in one participant with ALS reached 62 words per minute from four microelectrode arrays, 3.4 times the previous record, with a 9.1 per cent word error rate on a 50-word vocabulary and 23.8 per cent on 125,000 words.
3 · Frontier questions
Frontier The architectural shift of the last decade is closed loops, not stronger stimulation. Adaptive DBS, responsive neurostimulation and the brain-spine interface share one design: real-time state estimation feeding a controller fitted to one nervous system. Speculative A minority reading takes this further and says plasticity engineering is really controller engineering — that the biology has not moved, and every gain since 2015 came from better control, personalisation and timing. The same shape appears outside the skull: Slade and colleagues personalised ankle exoskeleton assistance in the real world in 32 minutes against 128 minutes for metabolic-cost methods, cutting energy per distance by 17 ± 5 per cent. Speculative The hypothesis is testable and currently winning on points: if controller improvements keep producing gains while pharmacology does not, the biological substrate is not the limiting factor. Its falsifier is a drug that moves an outcome no controller can.
Frontier The brake is druggable in principle and undruggable in practice. The C6S result is a genuine proof of concept — restoring one sulphation state rescued both LTP and memory in aged animals — but chondroitinase ABC is a bacterial enzyme with no systemic human delivery route, and the alternative arm of that same experiment was an AAV injection. Speculative Delivering a human sulphotransferase rather than a bacterial enzyme is the more interesting half of the proposal: let the animal retune its own matrix. Frontier What that requires is CNS gene delivery at scale, and the fetched record is unambiguous about what CNS delivery currently costs. uniQure's AMT-130, the largest CNS gene-therapy signal on record, reaches the striatum by MRI-guided convection-enhanced stereotactic injection — neurosurgery, per patient, per dose.
Frontier Paired stimulation might be a general-purpose plasticity tool, and nobody has shown it outside motor rehabilitation. The VNS-REHAB mechanism — neuromodulator release timed to a behaviour, driving Hebbian consolidation — is in principle trait-agnostic. Speculative For the general version to hold, three things must be true that are not yet shown: that pairing works for non-motor learning, that closed-loop timing can be achieved without an implant, and that the tripling of corticospinal connectivity the manufacturer claims generalises to circuits that are not corticospinal. The first is the cheapest to test and nobody has published it.
Speculative Psychoplastogens without hallucinogenesis are the field's most-pursued molecular goal and rest on an unestablished separability assumption. Several programmes aim to keep the TrkB and mTOR plasticity signal while removing 5-HT2A-mediated perceptual effects. Frontier Calder and Hasler's own statement that the pathway downstream of 5-HT2A is not fully characterised is precisely why separability cannot yet be asserted: you cannot reliably subtract a component of a mechanism you have not resolved. Speculative There is also a harder possibility that the field mostly declines to state, which is that the subjective experience is not a side effect of the plasticity but part of the behavioural half of the pairing — the content that gets consolidated. If that is right, a non-hallucinogenic psychoplastogen is a drug with its training removed.
Frontier The valproate result's most informative property is its non-replication. Thirteen years, and the field's flagship human critical-period-reopening finding has not been repeated with a baseline measure, a parallel-group design or a durability follow-up. Speculative That says more about the field than about the result. The study is cheap: a licensed generic anticonvulsant, a training task that runs on a laptop, and a few dozen participants. Its absence from the literature is a statement about incentives, not about feasibility.
Speculative The strongest sceptical position is that there is no headroom, and it is the honest terminal position. Owen's transfer effect sizes of 0.01 to 0.22 and Draganski's explicitly transient structural change together suggest that an adult brain reallocates capacity rather than creating it, and that engineered plasticity will therefore always trade one competence for another. Speculative What would falsify it is a demonstration of durable generalised gain — not task gain, not restored function — in a healthy adult. No such demonstration exists in the fetched record. Speculative Note the shape of the counter-evidence that does exist: Lorach's participant gained function he had lost, not function he never had, and gained it in the specific domain he trained for 15 weeks.
Frontier Two questions this brief deliberately leaves open. Whether the adult human hippocampus generates new neurons remains contested; the 2018 papers that reached opposite conclusions and the 2025 result that partly resolves them could not be fetched for this brief, so nothing is asserted about the current state of that question. Frontier And whether single-session transcranial direct-current stimulation has any reliable cognitive effect in healthy people rests on a 2015 quantitative review that was located but not read here; its numbers are not quoted, and the claim is carried only as a caution against citing tDCS as a non-invasive equivalent of the implanted work.
Speculative The single most tractable-looking gap in the whole subject is a measurement, not a therapy. There is no human biomarker for perineuronal-net density or chondroitin sulphation state. Every human plasticity intervention is therefore administered open-loop, into a brain whose plasticity state is unknown before, during and after. Handwave A PET ligand or CSF assay for net state would convert the field from open-loop to closed-loop in one step, allow dose-finding against a mechanism rather than against a behaviour, and make the durability question answerable. Nobody in the fetched record is building one.
4 · Technological bottlenecks
Established The workback plan to a durable, general enhancement of adult neural plasticity has seven links, and the binding one is not the drug. They are stated here in dependency order, with the constraint type on each.
Speculative Link 1 — a human biomarker for plasticity state. A PET ligand or CSF assay for perineuronal-net density and C6S sulphation, validated against post-mortem histology. Scientific unknown, and nobody is working on it. Speculative Link 2 — a systemically deliverable brake-release agent. A small molecule or biologic that changes net chemistry in human cortex. Chondroitinase ABC is bacterial and has no systemic route; the AAV alternative is neurosurgical. Scientific and industrial. Frontier Link 3 — show the reopened window is usable. Replicate the valproate absolute-pitch result with a proper baseline, a parallel-group design and durability follow-up. Scientific unknown, and thirteen years without a replication attempt.
Frontier Link 4 — show the gain outlasts the intervention. Any human plasticity intervention measured at least twelve months after withdrawal. Lorach 2023 is the only fetched example and it is one man. Speculative Link 5 — show it generalises beyond one trained domain. A pre-registered trial with a transfer effect size above 0.3, against a prior set by 11,430 people at 0.01 to 0.22. Speculative Link 6 — show it is safe to destabilise adult circuits. Long-term follow-up specifically powered for maladaptive plasticity: amblyopia-analogue effects, seizure risk, personality change. Scientific and institutional. Established Link 7 — non-surgical delivery. Everything that works — adaptive DBS, responsive neurostimulation, the brain-spine interface, paired vagus stimulation — is implanted, and the non-invasive candidate has no reliable healthy-subject effect. Industrial capability.
Speculative Link 5 is binding, and it is a scientific unknown with a strongly negative prior. Every other link has at least one positive demonstration somewhere in the record: the brake is releasable in animals, a window has been reopened once in humans, a gain has outlasted a device in one person, an implanted pairing has been approved. Transfer has nothing. Speculative That asymmetry should govern how the field is funded. The temptation is to work on link 2 because it is a chemistry problem and chemistry problems feel tractable; the value is in link 1, because without a biomarker no result at any other link can be interpreted, and in link 5, because a solved link 2 with an unsolved link 5 produces an expensive drug that makes people better at one thing.
5 · Research dependencies
Established This subject cannot produce its own delivery technology and has to inherit it. The fetched CNS-delivery record offers three templates and each carries its own tax. Antisense oligonucleotides reach the central nervous system by intrathecal injection and do so at real scale — nusinersen for spinal muscular atrophy from December 2016, tofersen for SOD1-ALS in 2023 — but they act on RNA, which suits a knockdown and not a matrix-remodelling enzyme. Frontier AAV reaches deep structures by MRI-guided convection-enhanced stereotactic injection, which is what AMT-130 requires per patient. Frontier Lipid nanoparticles carrying mRNA deliver a foreign protective protein to a defined tissue with local and transient expression, as Kirtane and colleagues showed for a tardigrade damage-suppressor protein in epithelium — the closest existing template for a transiently expressed plasticity agent, and it has not been shown to cross into cortex.
Established It also depends on the fields that bound what plasticity could possibly move. Neurogenetics supplies the ceiling: common-variant SNP heritability for intelligence of 0.19 with out-of-sample polygenic prediction up to 5.2 per cent, and 1,578 of 3,144 brain-imaging phenotypes showing significant SNP heritability. Those numbers are the reason serious people look to plasticity rather than to genotype for adult cognitive change, and they are also the structural substrate any plasticity intervention has to move. Established Bioelectric Medicine and Human-Machine Symbiosis supply the hardware and the control theory, since every working intervention here is an implanted closed loop. Speculative Precision Medicine supplies the missing discipline: this field has no stratification, no responder biomarker and no companion diagnostic, which is exactly the failure mode that field exists to name.
Established And it depends on a workforce, which is the dependency nobody lists. The behavioural half of every result here is delivered by a trained human: a rehabilitation therapist for the approved device, a psychological support pair for the psychedelic protocols, a supervised training block of 40 sessions for the brain-spine case. Speculative That is a supply constraint of the same kind as a reagent or a fab, and it is the one that does not fall with scale. A plasticity programme that succeeds scientifically and does not plan for it will discover its rate limit in staffing rotas rather than in chemistry.
6 · Required experiments
Frontier Six experiments, in the order that would extract the most information per pound.
Frontier One: replicate valproate properly. Parallel-group rather than crossover, a genuine pre-training baseline, a pre-registered primary endpoint on the pitch task, and re-testing at three, six and twelve months after withdrawal. Roughly 120 participants, a generic anticonvulsant and a laptop task. It is the cheapest decisive experiment in this brief and its absence for thirteen years is the field's most conspicuous unforced error. Speculative Two: build the biomarker. Screen candidate PET ligands against aggrecan and chondroitin sulphation epitopes, validate in post-mortem human cortex against histological net counts, then test-retest in living volunteers across the lifespan. Without this, every other experiment reports a behaviour and infers a mechanism.
Speculative Three: pair stimulation with a non-motor behaviour. Take the approved vagus-stimulation pairing protocol and apply it to a perceptual or linguistic discrimination task in a population already implanted for another indication. This tests trait-agnosticism at near-zero marginal risk, and the patient population exists. Frontier Four: measure durability directly. Any of the existing interventions, followed for at least twelve months after withdrawal, with the primary endpoint being retention rather than acquisition. No fetched human study in this subject has a twelve-month post-withdrawal endpoint except the single-participant brain-spine case.
Speculative Five: run the transfer trial that the field keeps not running. Pre-registered, adequately powered, with transfer effect size as the primary outcome and a target of 0.3, against a training-only comparator that receives the identical behavioural protocol. If a plasticity agent cannot beat 0.3 with the training held constant, the no-headroom hypothesis stands. Handwave Six: test the harm hypothesis on purpose. In animals, reopen the window and then deliberately deliver a degraded input — the deprivation-amblyopia analogue — and measure how much easier maladaptive learning becomes. Nobody wants this result, which is why it should be produced before humans are dosed rather than after.
Established One design rule runs through all six. Because the training is half the therapy, the comparator must receive it in full. A plasticity trial whose control arm gets a placebo and no training is not measuring the drug; it is measuring the training, and the record already tells you the training works — trained-task effect sizes up to 1.63. Frontier The approved device is the only intervention in this brief that has been tested that way, with both arms doing the rehabilitation, and its measured effect is correspondingly modest: roughly a doubling of the responder rate rather than a transformation.
7 · Engineering requirements
Established Every intervention in this subject that works is implanted, and the hardware requirements are precise. The brain-spine interface used two 64-electrode cortical arrays plus a 16-electrode epidural paddle and an implantable pulse generator, with roughly six months of calibration before the 15-week rehabilitation block. The speech neuroprosthesis used four intracortical microelectrode arrays across two cortical areas. Paired vagus stimulation requires a cervical electrode and a chest generator, and a therapist with a trigger. Established Adaptive DBS shows where the engineering effort now goes: the sponsor's headline feature is not a new electrode but a programming aid that cuts initial parameter search by 85 per cent, because fitting the controller is the expensive part.
Speculative The non-invasive equivalent does not exist and its most-cited candidate is a caution rather than a route. Transcranial direct-current stimulation is the obvious substitute and the quantitative review most often invoked against it was not read for this brief, so no numbers are given; what can be said is that no non-invasive method in the fetched record produces a plasticity effect comparable to any implanted one. Speculative Until that changes, the engineering requirement for population-scale plasticity engineering is a surgical throughput the health system does not have — and a drug route exists precisely because implantation does not scale.
Speculative A useful analogy sits outside neuroscience entirely. Personalised exoskeleton assistance moved from 128 minutes of metabolic-cost optimisation to 32 minutes of real-world personalisation, delivering 17 ± 5 per cent metabolic savings, 23 ± 8 per cent on a treadmill and a 9 ± 4 per cent increase in self-selected speed. Frontier A later simulation-trained controller reported larger reductions — 24.3 per cent walking, 13.1 per cent running, 15.4 per cent stair-climbing — and carries a Nature editor's note dated 27 July 2026 recording concerns about the availability of data and code supporting those results, which is part of the record and is cited here with it. Speculative The transferable lesson is that in a human-machine loop, personalisation time is the binding engineering variable, and it fell by a factor of four without any change to the biology.
8 · Adjacent technologies
Established Closed-loop neuromodulation is the adjacent technology that has already arrived. Responsive neurostimulation for epilepsy has been cleared since 2013 and adaptive deep brain stimulation since February 2025; both are controllers, and both were built by device companies solving a Parkinson's or epilepsy problem rather than a plasticity problem. Any plasticity intervention that needs timing rather than dose inherits their hardware and their regulatory precedent.
